Young Adult Services

COMMUNITY HOUSING

The Young Adult programs provide 24-hour on-site staff support to young adults, ages 18-25, transitioning out of Department of Children and Family (DCF) services or other transitional living situations. Programming specializes in supported housing to individuals who are DMHAS-eligible for Young Adults Services and includes individualized service planning, life skills training, case management, education and vocational supports, psychoeducational groups, and socialization. 
 
A special focus is placed on coordination between DMHAS, DCF, and local school systems to ensure that individuals complete their education and gain the skills necessary to follow their individual vocational interests. Fully furnished apartments are provided with private bedrooms and shared kitchens, bathrooms, and living rooms. Individuals in the Independent Community Living program live in their own apartments with 16-hour or 24-hour staffing based on their needs. 

Continuum also operates a respite bed at our Crisis/Respite Program in New Haven for a young adult in need. Referrals are made through, but not limited to, Young Adult Service at the West Haven Mental Health Clinic.
