Continuum of Care, Inc.’s mission is to enable people who are challenged with mental illness, intellectual disabilities, addiction, or homelessness to rebuild a meaningful life and thrive in the community.
Recovery Transformation Health & Wellness
Continuum's philosophy on wellness is based on the Eight Dimensions of Wellness as defined by SAMSHA
Sadly, people with serious mental illness die, on average, 25 years earlier than the general population due to chronic medical conditions like cardiovascular disease, diabetes, and obesity typically caused by lifestyle and medications.
Continuum of Care’s Recovery Transformation Health & Wellness Team teaches and utilizes holistic, all-natural practices in each of the Eight Dimensions of Wellness. They offer unique interactive workshops, exercise activities and healthful eating initiatives to prevent or reduce these serious health conditions and set people on the path to better physical and mental health.
The Team conducts individual and group sessions as well as access to educational and vocational services, support for self-advocacy, health and well-being activities, relapse prevention, financial empowerment learning, connection to spiritually based groups/church, and assistance with developing a natural supports system. They support each participant in achieving their unique needs and goals. Monthly group activities include low-impact exercise, walking, hiking, ladder toss, cornhole, tennis, bowling, African drumming, artistic expression, music, and healthy meal preparation/instruction.
The goal is to collaboratively build up and empower each individual to achieve their best health and wellness.
